Haircut mitral valve repair: posterior leaflet-plasty.
نویسنده
چکیده
A 63-year-old man presented with symptomatic severe mitral regurgitat ion. Both trans-esophageal 3-D echocardiography and operative linear measurements showed a 25 mm long A2 anterior leaflet with a very wide (23 mm) prolapsing 30 mm long P2 posterior mid-scallop (Figure 1A,B). P1 and P3 were diminutive in height and width (Figure 2). A ‘haircut’ posterior leaflet-plasty was done to maintain leaflet-annular integrity, shorten and reduce the prolapsing P2 and preserve adequate tissue for coaptation.

LA R arge Annuloplasty Rings Facilitate Mitral Valve epair in Barlow ’ s Disease avid
p o r a c Background. Excess leaflet tissue in Barlow’s disease redisposes patients to left ventricular outflow tract bstruction and residual mitral regurgitation after miral valve repair as a result of systolic anterior motion f the anterior mitral leaflet. In addition to convenional repair techniques such as sliding plasty and eaflet shortening, our approach in Barlow’s disease as included th...

We report the 20 year follow up of a female patient who underwent mitral valve billowing and prolapse (Barlow) repair at the age of 29 yrs. Our repair method to aggressively remodel the valve, which maximizes predictable anatomic and physiologic efficacy and minimizes unpredictable results incorporates a complete resection of the posterior leaflet, a sliding and folding plasty with the remainin...
متن کاملExperimental stenting of the posterior mitral leaflet to correct prolapse in mitral valve insufficiency.
OBJECTIVE This study investigated the use of a new concept of mitral valve reconstruction using a novel device to stent the posterior mitral leaflet in combination with semicircular annuloplasty. Modern mitral valve repair is an accepted modality and a routine procedure for treatment of degenerative mitral valve insufficiency. One of the most common mechanisms of mitral valve insufficiency is l...
متن کاملCan three-dimensional echocardiography accurately predict complexity of mitral valve repair?
OBJECTIVE Feasibility of mitral repair is a key factor in the decision to operate for mitral regurgitation. Repair feasibility is highly dependent on surgical experience and repair complexity. We sought an objective means of predicting complexity of repair using three-dimensional (3D) transoesophageal echocardiography. METHODS In a cohort of 786 patients who underwent mitral valve surgery bet...
متن کاملSustained improvement after combined anterior mitral valve leaflet retention plasty and septal myectomy in preventing systolic anterior motion in hypertrophic obstructive cardiomyopathy in children.
OBJECTIVE Anatomic alterations of the mitral valve such as increased mitral leaflet area, length and laxity, and anterior displacement of the papillary muscles in hypertrophic obstructive cardiomyopathy predispose patients to residual systolic anterior motion and persistence of outflow obstruction and mitral regurgitation after septal myectomy. We investigate the long-term results of combined a...
